7. Is this the bride first marriage?(if not divorced? widowed?)
8. Is the bride a convert?
9. Are the fathers alive?
10. Are any of the fathers Cohanim or Leviim?
11. If the text is Orthodox or Conservative then please ask the Rabbi whether I should leave out the leg of the letter "kuf" in the word uknina or not.
12. Will mothers names be included in the ketubah? (Usually only the bride and groom father names are mentioned and not the mother. However, sometimes, if the couple wants the mother name mentioned the Rabbi will allow it, so if yes, please check withe Rabbi first if this is acceptable to him) if yes: Groom mother name(s): Bride mother names(s):
13. Will family names be included in the ketubah? (Traditionally family names are not mentioned in the ketubah but lately this is changing. Please check with the Rabbi whether he wants/permits it).if yes: groom family namebride family name(include spellings if possible)
14. If any names are not common (or are Yiddish names), or have unusual spellings, then please include a spelling in English characters for the name (e.g. Shayne - shin, yud, yud, nun, ayin)
